Two cannulas can carry the same three openings and behave differently, because the order along the shaft decides which opening meets new tissue first. Leading with the parallel pair spreads the draw across the front of the stroke instead of concentrating it at one point.

How the Port Order Works

Three openings, arranged with the two parallel ports nearest the tip and the single central port set behind them. The count is the same as a central-port-first cannula; the order along the shaft is reversed, and that order is what changes how the instrument behaves in a pass.

With the pair leading, the widest part of the aperture reaches new tissue first. As the cannula advances, the two parallel ports open the plane across their span and the central port behind them works tissue that has already been engaged. Removal therefore starts wide and finishes narrow on every forward stroke, which spreads the draw across the front of the pass instead of concentrating it at a single leading opening.

The practical effect is a smoother entry into untreated tissue. A single leading port bites at one point and can pull the cannula toward it; two leading ports share that load between them, so the instrument tracks straighter as it advances and is less inclined to dig at the start of a stroke. Surgeons who work in long forward passes tend to prefer this order for that reason.

The trailing central port then acts as the finishing opening of the stroke, taking the last of the tissue the pair has opened. Total removal is roughly three times a single hole, held within a compact group so the surgeon can still judge how much has come from any one area. The distal tip is closed and rounded, separating the plane by blunt dissection while the vacuum works through the three ports rather than the end of the shaft.
