Sickle Ear Knives: Schuknecht, Wullstein and Tabb

A sickle knife has a short, curved blade with a sharp point, shaped like a tiny sickle. The point starts a cut exactly where the surgeon wants it, and the curved edge draws cleanly through thin skin and tissue.

In ear surgery it is used to make incisions in the ear canal skin, to freshen the edges of an eardrum perforation before a graft is placed, and to separate fine tissue and adhesions in the middle ear.

Healermed makes three sickle patterns: the angled Schuknecht (14.5cm), Wullstein (15.5cm) and Tabb (15.5cm). They do the same job and differ in blade shape and length. Most surgeons choose the pattern they trained with, and many keep two.

Schuknecht Roller Ear Knives: Vertical and Horizontal

A roller knife has a small round blade at the tip. The blade cuts along its whole edge, so it is drawn along the ear canal skin to make long, even incisions without the snagging a pointed blade can cause.

It is used to make the incisions in the ear canal skin that are needed to lift the skin and eardrum flap (the tympanomeatal flap) at the start of middle ear surgery.

The Vertical and Horizontal models differ in how the blade is set on the shaft, so each suits a different direction of cut in the ear canal. Most surgeons keep both.

Rosen Ear Knives: Straight, 45 and 90 Degree

A Rosen knife has a small round blade with a semi-sharp edge. It is made to lift (elevate) the ear canal skin and the eardrum away from the bone, separating tissue gently where a sharp point would tear it.

The blade is made straight, at 45 degrees and at 90 degrees. The straight blade works in line with the view; the angled blades turn to work around the curve of the ear canal and under the edge of the flap.

Each angle is made in ten blade diameters: 1.0, 1.4, 1.5, 1.6, 2.0, 2.3, 2.5, 2.6, 2.8 and 3.0mm. A smaller blade fits narrow canals and fine work near the ossicles; a larger blade lifts more tissue with each stroke. Many surgeons keep three or four diameters in each angle.

Plester Mucosa Ear Knife

The Plester mucosa knife is made for cutting and lifting mucosa (the thin lining of the middle ear) and skin in ear surgery.

It is used where a thin layer has to be cut and raised cleanly without damaging what lies underneath, such as raising mucosa and skin flaps in the middle ear and ear canal.

Sexton Ear Knives: Straight, 45 Degree and Bayonet

Sexton ear knives are long ear knives, 17 to 18cm, for incisions in the ear canal skin and eardrum made through an ear speculum. The long shaft keeps the hand well back from the ear.

The straight knife (18cm) cuts in line with the view. The 45 degree bent knife (17cm) turns the blade to reach cuts at an angle. The Sexton-Gerzog bayonet knife (17cm) has an offset, bayonet-shaped shaft that keeps the hand and handle out of the line of sight through the speculum or microscope.

Care and Sterilization

Ear knife blades are fine and lose their edge if they touch metal. Use them on soft tissue only, never on bone, wire or needles, and store each knife with its blade in a tip guard so the edges never knock against other instruments. A dull blade tears tissue instead of cutting it, so check the edge under magnification at every cycle. Clean the instrument straight after use, before blood and tissue dry on the tip. Use a soft brush and an ultrasonic cleaner, rinse, and dry fully. Put a drop of instrument lubricant on the joint before sterilizing. Steam autoclave at up to 134°C in a tray with a silicone mat or tip guard, so the fine tip never knocks against other instruments. Check the tip under magnification at every cycle.
