The Luer lock is the standard connection for fat injection. A threaded collar screws onto the syringe so the joint holds while the surgeon presses the plunger. On an injector that matters more than on an aspiration cannula: injection pressure pushes outward at the hub, and a slip fit can separate mid-pass and lose both the graft and the plane.

How Two Openings In Line Place Fat

Two openings, both on the same side of the shaft and set in one straight line. As the cannula withdraws, each opening lays its own thread, and because they share a side both threads go the same way.

That is the difference from a single hole injector. Two deposits per pass cover a recipient area in roughly half the passes, which matters when the graft volume is substantial and the number of withdrawals is what limits the case. The threads sit parallel and close, so the fill is even rather than lumpy.

Keeping both openings on one side preserves the aiming that makes an injector controllable. Rotate the shaft and both threads redirect together, deep or superficial, exactly as with a single opening. Openings on opposite sides would place fat in two planes at once and take that choice away.

Two rather than more is the restraint. The surgeon still knows what was placed on each withdrawal. The closed blunt tip separates the plane without dividing vessels or nerves.

How an Injector Cannula Places Fat

An injector cannula is the delivery end of a fat transfer. The syringe supplies the pressure, but where the graft ends up, and in what form, is decided by the cannula: its diameter, its opening, and the way the surgeon moves it.

Placement is done in retrograde. The cannula is advanced through the recipient plane, and fat is laid down as it is withdrawn, so the graft fills the tunnel the cannula has just made rather than being forced into tissue ahead of it. Each pass deposits a thread of fat. A result is built from many of those threads, crossed and layered, not from a few large boluses.

That is why the opening matters. Its size sets the diameter of the thread laid down; its position sets which way the fat is directed as the cannula withdraws; and its edge decides whether the parcel passes cleanly or is sheared at the exit. A wider opening places more per pass and fills faster; a smaller one places less and gives finer control in thin tissue where the graft can be seen through the skin.

Diameter and length work with it. A narrower cannula makes a smaller tunnel and allows finer layering, but it also raises injection pressure for the same fat, so the surgeon pushes harder. A longer shaft covers more of the recipient area from one entry point, which means fewer punctures.
