Anaerobic fat transfer adapters connect two syringes so harvested fat can be transferred and emulsified in a closed system, without exposure to room air. They are available in seven bore sizes from 1.0mm to 3.0mm, so the surgeon chooses how much the graft is sheared on each pass. Two syringes and a connector is the whole apparatus for preparing a graft. Which connector goes between them decides whether the surgeon ends up with structural fat, microfat or a nanofat emulsion.
The adapter is the connector between two syringes. Fat is driven from one barrel through the adapter into the other, and back again, and everything that happens to the graft in that process happens inside the adapter.
Anaerobic means the path is closed. The graft goes from syringe to syringe through a sealed channel without being decanted, poured or exposed to room air at any point. That matters because harvested fat is living tissue: every transfer into an open bowl is an opportunity for contamination and for the tissue to sit exposed while the surgeon works. A closed transfer keeps the graft in a sealed system from harvest to injection.
The bore is the working dimension. Fat forced through a narrow channel is sheared, and the narrower the channel the more the parcels break down on each pass. A 1.0mm bore emulsifies aggressively and reaches a nanofat consistency quickly; a 3.0mm bore moves the same graft between syringes while leaving the parcels largely intact. The adapter is therefore not just a connector, it is the instrument that decides what grade of graft the surgeon ends up with.
That is why the range exists. Structural grafting needs intact parcels and a wide bore; skin quality treatment needs an emulsion and a narrow one; microfat sits between them. Using one bore for every case means accepting whatever grade it produces.

The adapter is made from German stainless steel and is fully reusable, steam autoclavable to 134°C. Flush the bore from both ends immediately after every case, before residue sets, and hold it to the light to confirm the channel is clear end to end. A partly blocked bore raises the force needed on the plunger and shears the graft more than the size on the adapter suggests, which is a failure that gives no outward sign. Check the Luer threads at both ends at every cycle, since a worn thread leaks under the pressure a narrow bore generates.

What does anaerobic mean on a fat transfer adapter?
It means the transfer path is closed. Graft passes from one syringe to the other through a sealed channel, without being decanted into a bowl or exposed to room air. Which bore size should I use?

Is a narrower bore always better?
No. A narrower bore applies more mechanical stress to the tissue on every pass. That is what you want when preparing nanofat and what you want to avoid when placing structural graft. Choose the widest bore that produces the grade the case requires.
