Luer lock is a standard, but manufacturing tolerances differ between brands, and a hub cut to the tight end of the tolerance band can seat poorly on some syringes. A universal hub is machined to sit centrally in that band, so it locks properly across whatever stock is on the shelf that week. Under vacuum a poor seat is not just annoying, it leaks.
Three ports arranged as an inverted triangle: two openings sit above, side by side, and a single port sits below them. It is the pyramid pattern turned over, and the reason to turn it over is which way the aperture is weighted.
On a standard pyramid the single port sits at the top and the pair below, so the widest part of the aperture faces down toward the deeper plane. Reversing it puts the pair uppermost, so the broadest draw faces the skin side when the cannula is held in the normal working orientation. That makes the reverse pyramid the superficial member of the pair: more of the aperture is working the layer the surgeon can see and feel.
The single port below then balances the pull. Without it, two openings on one side would drag the liposuction cannula toward the skin. The third port underneath shares the vacuum load across the shaft, so the instrument tracks straight instead of climbing, which is exactly what you want when the aperture is weighted toward a thin layer.
