A fixed handle removes the weakest point of a modular cannula. On a pyramid shaft it also fixes the pattern orientation permanently, so the surgeon can tell which way the apex hole is pointing from the handle alone, without stopping to look at the shaft.
The pyramid pattern is three openings arranged as a triangle on the shaft: one hole on the upper surface, and two more set parallel below it. The name comes from that layout — a single apex with a base of two. It is a deliberate middle setting between a single hole and a full multi-port shaft.
The upper hole is the one the surgeon aims. Held with the apex facing the layer being worked, it behaves much like a single hole cannula: removal happens at a point the surgeon can see and control. The two parallel holes below add draw without spreading it along the length of the shaft, so the cannula clears more per pass than a single hole design while the working aperture stays compact.
The other effect of a triangle is balance. With one opening the whole vacuum load sits on one side and the cannula can pull toward it; three openings spaced around the shaft share that load, so the instrument tracks straighter through the tissue and needs less correction from the hand across a long pass.
The distal tip is closed and rounded. It separates the plane by blunt dissection as the cannula advances, pushing vessels and nerves aside rather than dividing them, and the vacuum works through the three side holes rather than the end of the shaft.

The cannula is made from German stainless steel with a passivated surface that resists corrosion and pitting through repeated cleaning cycles. It is reusable and can be steam autoclaved at 134°C. Flush the lumen and all three holes with clean water immediately after use and dry fully before autoclaving. Check each opening under light at each cycle: the pyramid pattern only behaves predictably while all three holes draw evenly, and one partly blocked opening shifts the balance without the surgeon noticing.
A fixed handle serves one shaft only, so it suits sizes you use case after case; if you rotate through many diameters, a detachable fitting costs less per size. Diameter sets how much fat comes out per pass and length sets the reach. For general body contouring of the abdomen, flanks and thighs, the 3.0mm to 4.0mm diameters in 25cm to 35cm are the usual working range. The 2.0mm to 2.5mm range in 15cm to 25cm covers arms, the neck and mid-stage contouring, which is where the pyramid pattern is chosen most often.
