The House-Dieter malleus nipper, up cutting, is a fine bone-cutting instrument for middle ear surgery. It is used to cut the head of the malleus, the first small bone of hearing. What a Malleus Nipper Does

The malleus is the first of the three small bones in the middle ear. In some operations part of it has to be removed: to open the view into the attic in cholesteatoma surgery, to free a malleus head that is fixed to the bone around it, or to make room when rebuilding the chain of hearing bones (ossiculoplasty).

A malleus nipper cuts this bone cleanly in a single controlled bite. The small 1.3mm jaw cuts only what it holds, so the surgeon can take the bone off in precise steps instead of drilling close to the other ossicles.

Why Up Cutting

The cutting jaw opens and cuts upward. It is used when the part of the malleus to be cut lies above the line of the shaft. The direction you need depends on the ear being operated on and how the malleus sits in the view, so the four directions are often bought as a full set.

The other directions are down cutting (HM-MN-D13), right cutting (HM-MN-R13), left cutting (HM-MN-L13).

Care and Sterilization

Use the nipper on bone only. Never cut wire, prosthesis material or needles with it — that nicks the edges. Keep the jaws closed in a tip guard during storage. Clean the instrument straight after use, before blood and tissue dry on the tip. Use a soft brush and an ultrasonic cleaner, rinse, and dry fully. Put a drop of instrument lubricant on the joint before sterilizing. Steam autoclave at up to 134°C in a tray with a silicone mat or tip guard, so the fine tip never knocks against other instruments. Check the tip under magnification at every cycle.
